Section 504 Assessment and Transition Plan
A Section 504 Assessment and Transition Plan is a planning and compliance report required by HUD and the office of Fair Housing and Equal Opportunity (FHEO). The report assesses a development for compliance with applicable accessibility requirements – Uniform Federal Accessibility Standards (UFAS), Americans with Disabilities Act (ADA), and Fair Housing Act (FHA). The Assessment outlines the requirements, any deficiencies observed, and possible corrective measures. Estimated costs and recommended schedules for the corrections are also provided. Tenant forms, lease documents, and other management policies are also reviewed for compliance with accessibility requirements. CHS can provide staff training on accessibility policies and standards. CHS can also facilitate a meeting with the housing provider and local accessibility advocates.
